Alzheimer’s clues found in Terry Pratchett’s Discworld
Years before Terry Pratchett went public with his Alzheimer’s diagnosis, the Discworld author may have been leaving quiet clues on the page.
A new linguistic study suggests Pratchett’s writing style began to subtly shift almost a decade before doctors diagnosed him with posterior cortical atrophy, a rare form of early-onset Alzheimer’s, in 2007. By tracking how his language changed across the long-running Discworld series, researchers say they can see signs of the disease emerging far earlier than anyone realized.
The team, led by Dr. Thom Wilcockson of Loughborough University, analyzed 33 Discworld novels, looking at what they call “lexical diversity”—mainly the variety of nouns and adjectives Pratchett used over time. Their findings, published in the journal Brain Sciences, point to a marked dip in linguistic complexity around the 1998 novel The Last Continent.
That drop appears almost ten years before Pratchett’s official diagnosis. Even after adjusting for book length, co-author Melody Pattison of Cardiff University notes, the decline in lexical variety remained “significant”—though still too subtle for the average reader to notice. For fans, The Last Continent is hardly a creative collapse; it’s more like a barely audible crack in an otherwise roaring, joke-packed voice.
Because Pratchett kept returning to the same fictional universe for more than 40 books, his work has become an unusually rich dataset for dementia research. A continuous, decades-long record of one writer’s evolving style is rare in any genre, and almost unheard of at fantasy’s blockbuster scale. It’s the same sprawling fictional world that now powers new tabletop projects, from Modiphius’ games and shows to a growing wave of Discworld RPGs and board games.
Posterior cortical atrophy, the Alzheimer’s variant Pratchett lived with, particularly affects visual and language processing. That makes written output—novels, emails, even social media posts—a promising place to look for early changes. Loughborough’s researchers argue that the shift they observed in Pratchett’s prose likely reflects the disease’s “preclinical phase,” when the brain is already being altered but outward symptoms have not yet become obvious.
“It may enable us to use interventions sooner before the brain is damaged beyond repair,” Wilcockson said in a university release. If similar patterns can be picked up in other writers—or in ordinary patients’ everyday language—lexical analysis could become a low-cost screening tool to flag people who need further testing.
Pratchett, who died in 2015 at 66, spent his final years campaigning publicly for dementia awareness and the right to die with dignity. For readers, this research adds a poignant new layer to his legacy: the possibility that even as he built one of fantasy’s most beloved worlds, the disease he later fought so fiercely was already whispering through his sentences.
It also lands at a moment when Pratchett’s influence feels newly alive, from Rhianna Pratchett’s confirmation of a fresh Discworld screen adaptation to games and conventions that keep Ankh-Morpork’s chaos spinning. Now, his books are doing something else: helping scientists understand how stories—and the language that shapes them—might one day help diagnose dementia sooner, and give real people more time inside their own worlds.
